Patibandla Dental Health Plan

Patibandla Dental is committed to providing dental care in Canton, Ohio, for all our patients. To help meet this goal, we offer the Patibandla Dental Health Plan, a in-office savings plan catering to patients who do not have traditional dental insurance. This helps you receive affordable dental care at a price that meets your budget.

What is the Patibandla Dental Health Plan?
Our annual plan is designed to make dental care affordable for both individuals and families. Covered individuals may include you, your spouse and your dependent children ages 17 and under.

Benefits include:

• No waiting periods
• No deductibles
• No claim forms to file
• No yearly maximums

Membership includes:

• 2 healthy dental cleanings per year at no additional charge.
• 2 routine dental exams per year at no additional charge.
• Routine X-rays as prescribed by the doctor at no additional charge.
• A deduction of 10% off regular office fees for any additional treatments needed.

Membership benefits begin immediately on the date of enrollment when the membership paid in full. As an annual membership plan, the Patibandla Dental Health Plan will remain active for 12 months, starting the date of enrollment.

Limitations and Exclusions:
Any benefits not redeemed during the 12-month enrollment will be forfeited, with no rollover.

Any services you are referred out of office for, including oral surgery, periodontics or endodontics, are not covered under the Patibandla Dental Health Plan and must be paid for by the patient.

If a patient becomes insured by a traditional dental insurance provider during the enrollment of the Patibandla Dental Health Plan, our in-office savings plan becomes null and void with no refund of membership plan fees or rollover of treatments or discounts.
